Do you need help?
Managing health conditions such as diabetes, high blood pressure, mental health concerns, or substance use disorder can be tough. Enhanced care management (ECM) can provide you with free extras to make your life healthier and easier. It’s easy to join, and you can quit any time.
You may be eligible if you:
- Experience homelessness with complicated physical or mental health issues.
- Are often hospitalized or visit the E.R., or have short-term stays in skilled nursing facilities.
- Struggle with serious mental illnesses or substance use disorders accompanied by complex social needs.
- Are at high risk of requiring long-term institutional care.
- Reside in a nursing facility and wish to return to your community.
- Were recently released from incarceration with physical or mental health complications, substance use disorder, brain injury, HIV, or pregnancy.
- Are a child or youth with complicated physical, behavioral, or developmental health needs, or recently released from incarceration.
Your care team may include a:
- Nurse care manager.
- Behavioral health care manager.
- Care coordinator.
- Community health worker.
Assistance
Your care team assists you — by phone, in-person, or sometimes at your location — with obtaining cost-free services, including:
- Primary care.
- Mental health support.
- Community-based long-term services and support.
- Developmental health.
- Oral health.
- Social services.
No change to existing benefits
You keep your current providers, but get help:
- Connecting to doctors and scheduling medical appointments (including transportation to and from).
- Developing a personalized care plan.
- Figuring out follow-up after you’re discharged from the hospital.
- Managing medication.
- Connecting to food assistance or other social services.
